Accommodation Agreement
I authorize the Office of Disability Services to discuss my needs and accommodations with University Departments. This consent shall remain in effect until revoked by me, in writing. I agree to follow the guidelines for accommodations/services indicated in the Student Handbook which I have received. I will notify DS if my disability status changes, and I understand that requesting accommodations other than those indicated may require additional documentation.
If requesting any of the accommodations listed below, carefully review the corresponding agreement:
- Testing Accommodations
- Tests Scheduled in the DS Test Center:
- Only tests that are administered in person on campus should be scheduled in the DS Test Center. The DS Test Center is located in Fretwell 230
- If eligible for testing accommodations in the DS Test Center, I will follow the CODE OF STUDENT RESPONSIBILITY (U.P. 406), and the CODE OF STUDENT ACADEMIC INTEGRITY (U.P. 407) at all times. I understand that personal items are not allowed in the testing rooms, that these rooms are monitored by video surveillance, and that computer activity in the Test Center is monitored. CELL PHONES ARE NOT ALLOWED IN THE DS TEST CENTER NOR IN ANY DS SATELLITE TESTING LOCATIONS, SUCH AS THOSE USED DURING FINAL EXAMS. I understand that students found cheating or in violation of the cell phone policy will be reported to their instructor(s).
- Online Testing: For tests administered online by the instructor that students can take anywhere (at home, in the Library, etc.), these tests will not be scheduled in the DS Test Center. Students should contact the instructor prior to the test date to be sure that applicable accommodations are in place. It takes time for instructors to set up accommodations in Canvas. Students should notify their instructor(s) and their assigned DS staff member before beginning the test if accommodations are not in place at test time.
- Tests Scheduled in the DS Test Center:
Scheduling Tests: Students are responsible for scheduling tests and finals via the DS Portal by the deadlines. If deadlines are missed, Disability Services cannot guarantee space.
* TEST DEADLINES:
- Tests, Mid-terms or Quizzes:
- Fall/Spring Semesters – Schedule at least 6 days before the test.
- Summer Semesters – Schedule at least 3 DAYS before the test.
- Final Exams: DS Finals Deadlines differ by semester and are posted. Visit the Test Scheduling section for our specific Finals deadline.
- During Finals: The high volume of students produces limited space in FRET 230, DS utilizes additional space outside of our office for final exams. Headphones and ear plugs are available to help reduce distraction. DS will be unable to guarantee space in FRET 230 or individual rooms during the final exam period as first priority will be given to testers with assistive technology.
- Online Testing: Remember for tests administered online by the instructor that students can take anywhere, students should contact the instructor prior to the test date to be sure that applicable accommodations are in place. It takes time for instructors to set up accommodations in Canvas. Notify instructor(s) and the DS staff member before beginning the test if accommodations are not in place at test time.

- Supplemental Class Notes: To activate this service, first I must request “supplemental class notes” on my Letter(s), and next I must request a Note Taker using the “Request a Note Taker” module in the DS Portal. Additionally, I will attend class, take my own notes, and notify DS of any issues related to the note taking service in a timely manner. I understand that I am not permitted to share these notes with other students in any form and that doing so is a violation of academic integrity.
- Attendance Flexibility: This accommodation provides reasonable flexibility with course attendance policies. This accommodation is intended to build in modest amounts of flexibility to course policies for students who experience periodic disability-related flare-ups that cause interruptions that are likely to impact attendance.
— Because course policies are set by faculty, I am responsible for understanding the attendance policy for each course in which I am enrolled.
— This accommodation also addresses test days. If multiple tests are missed due to disability-related absences such that flexibility is no longer reasonable, I and my faculty are encouraged to talk with my assigned DS staff member.
— I am responsible for notifying my professors and my DS staff member of each disability-related absence within 24-48 hours of the absence occurring.
— I am responsible for making up missed work in a timely manner, and I should talk with my faculty so that I understand their expectations for any due dates.
— Excessive absences can negatively impact grades. If my absences become excessive, I am responsible for talking with my professors and my DS staff member to determine my options. - Equipment/Classroom Aid Loan: Disability Services provides equipment and/or classroom aids for short term loan to UNC Charlotte students registered with our office. Such equipment or classroom aids may be loaned to students in accordance with their accommodations and for academic purposes only. I agree to abide by the following terms and conditions: (1) I am personally responsible for all equipment or classroom aids that I check out, and will return all loaned items within the specified date. (2) I am responsible for returning all loaned items to the Office of Disability Services. (3) Failure to return loaned items at the end of the loan period will result in a hold on my academic record. (4) Missing, stolen, or non-functioning items must be reported immediately to Disability Services. (5) I am personally responsible for any damaged and/or missing items loaned to me and will be charged for missing or damaged items loaned to me. I understand that I am personally responsible for repaying the Office of Disability Services the full replacement cost of the loaned item(s).
- Accessible Texbooks (e-text): I agree that I will not copy, reproduce, modify, or distribute the alternate format materials nor allow anyone else to do so pursuant to the requirements of the Copyright Act of 1976 as amended (17 U.S.C. 101 et seq.). Failure to abide by this is a violation of federal copyright laws and the CODE OF STUDENT RESPONSIBILITY (U.P. 406), and the CODE OF STUDENT ACADEMIC INTEGRITY (U.P. 407) and may result in disciplinary action.
- Record Class Lectures: Permission to record lecture provides access to course content for students who have disabilities that limit their ability to take notes effectively. Recordings are for personal study use only. Lectures recorded for this reason may not be shared with other people without the consent of the lecturer. Smartpen, Glean Note-Taking App, or digitally-recorded lectures may not be used against the faculty member, guest lecturers, or students whose classroom comments are recorded as part of class activity. Information contained in the recorded lecture is the intellectual property of the professor, protected under federal copyright laws, and may not be copied, broadcast, published, or quoted without the consent of the lecturer. This prohibition includes placing recordings or transcripts of recordings on any web page or the Internet. The student agrees not to record/release private information shared by classmates. Further, the student agrees to delete any audio recordings when they are no longer needed for academic work.
- PowerPoints: PowerPoints provided by instructors as an accommodation are for your personal study use only. PowerPoints provided for this reason may not be shared with others or posted online.
- Assignment Extensions: Extensions are appropriate for students who have a chronic physical or mental health disability with brief, periodic flare-ups that are unpredictable and occasionally impact assignment completion. This accommodation is meant to provide reasonable and modest flexibility to allow the student to make up the assignment or an equivalent/alternative assignment.
— As much as possible, I should notify my instructor and my assigned DS staff member prior to the assignment due date if an extension is needed. However, there are rare circumstances when this may not be possible, and I may need to request an extension on the assignment due date.
— Assignment requirements will vary depending on the course and per the instructor and the syllabus. Students should follow the syllabus, work ahead when possible, and make up missed work in a timely manner. It can be helpful for to work with the Instructor to understand expectations for any new deadlines when an adjustment is needed.
–Extensions do not apply automatically or without limit, and I am responsible for requesting the extension from the instructor each time this accommodation is needed.
— The nature and structure of the course will help guide what is reasonable. If concerns arise, instructors are encouraged to talk with the student and to contact the assigned DS staff member.
— Repeated requests for assignment extensions may negatively impact progress in the course. We encourage students to discuss expectations with their instructors.
— Instructors are not required to provide extensions on assignments that were due before the date of this accommodation letter. My accommodation is not retroactive.
- I understand that I need to meet with my instructor(s) to discuss my accommodations. For online courses, I need to email my instructor(s) to follow up about my accommodations.
- I understand that I may choose not to use any of my approved accommodations. By not checking approved accommodations, I’ve declined those accommodations and they will not appear on my Accommodation Letters.
- I understand that it is my responsibility to address any concerns or changes regarding my accommodations or disability needs in a timely manner by notifying Disability Services that I need to connect with my assigned DS staff member. If the situation is complex, I will schedule an Advisement appointment with my DS staff member to get further assistance.
- By requesting accommodations via the DS Portal, I understand that I have agreed to abide by the terms outlined in this agreement.
