DS Test Center Agreement

DS Test Center Student Agreement:

  1. Be on time. If you arrive late, that time will be lost and your end time will remain the same.
  2. The DS lobby has limited capacity, please be mindful. Wear a mask at your own discretion. Help reduce the spread of germs by using hand sanitizer before entering the Test Center. Hand sanitizer and masks are available at the Front Desk. If you need assistance, let us know.
  3. Food is not allowed in the Test Center because crumbs attract rodents and insects. Drink/water in the Test Center is allowed. If food or drink are part of your accommodations, please notify DS Testing Staff at check-in.
  4. All your test information will be provided to you in an envelope. Do not open the envelope until you are in your assigned seat. The cover sheet includes proctoring details, your seat assignment, and your student ID number.
  5. If you have questions during your test, alert DS Testing Staff. We will contact your professor via phone/email. We cannot guarantee that the instructor will respond in a timely manner. Please contact your instructor after the test if you have any unanswered questions or concerns. In addition, you can write any questions in the margins for the instructor to see later.
  6. At the conclusion of your test, place all materials in the envelope and return the envelope back to the DS Testing office. Return any borrowed pencils and calculators, and any scratch paper to the Testing. *Once the test materials are returned to staff, they cannot be returned to you. Be sure that you have written your name, student ID number (if applicable), section number, etc. on your test. Please ensure that all test pages/materials are included prior to submitting your envelope.
  7. Cell phones and other electronics are not allowed in the DS Test Center, including all Test Center satellite locations used during Final Exams.
