How to Submit Documentation
Getting Started
New Students: Submit Online via the secure DS Student Portal.
- Students new to Disability Services should upload their documentation and complete the brief online intake form.
- You must have have a NinerNet user name, password, and Student ID number to use the DS Portal.
- If your documentation isn’t ready when you complete the online form, you can indicate that it will be sent separately.
- Not submitting through the DS Portal may delay processing.
- Note: We encourage all students to complete the online intake form, as the information you provide is important to the review process. If documentation is received before your webform is completed, we will contact you to ensure you complete it.

- Include your UNC Charlotte student ID number and University email on your documentation. Missing this information will delay processing.
- Keep copies of all submitted documents for your records.
- Help Safeguard Documentation: Do not include your Social Security Number (SSN). Only your UNC Charlotte student ID number and University email address are necessary for processing.
