How to Submit Documentation
Getting Started
New Students: Submit Online via the secure DS Student Portal.
- To submit disability documentation, students new to Disability Services should use the DS Portal. You’ll need your NinerNet user name, password, and Student ID number to log in.
- Complete the brief online intake webform. This information is important to the review process. If your documentation is not ready yet, you can indicate it will be sent separately within the webform.
- You can then upload your documentation directly to the Portal. Please note that not submitting documentation through the DS Portal may delay the review process.
- If we receive your documentation before you have completed the online intake webform, we’ll contact you via your University email to remind you to complete it.

Important Information for All Submissions:
- Include your UNC Charlotte student ID number and University email on your documentation. Missing this information will delay processing.
- Keep copies of all submitted documents for your records.
- Help Safeguard Documentation: Do not include your Social Security Number (SSN). Only your UNC Charlotte student ID number and University email address are necessary for processing.
