Submitting Documentation
At the college level, it’s important for students to initiate the process with Disability Services themselves. Through the secure DS Student Portal, students new to Disability Services can upload their documentation and provide information about their disability in their own words by completing the brief online intake form.
New Students should Submit Online using the secure DS Student Portal. Students must have a NinerNET Student ID number.
- If you don’t have your documentation at the time that you complete the online intake form, you may indicate that your medical provider will be submitting it separately.
Not submitted through the DS Portal may delay processing.
Because the information students provide in the webform is important to the review process, we want students to complete the online intake form. When documentation is submitted prior to the student completing the webform, we will follow up with the student to connect them to the online intake form.

Important
- Include your student ID number and University email on your documentation.
- Not including your student ID number and University email address will delay processing.
- Students are encouraged to keep copies of any documentation they submit to our office.
Help Safeguard Documentation
Don’t include your social security number on any documents submitted to Disability Services. Only your UNC Charlotte student ID number is necessary for processing.
