How to Submit Documentation

Submission Instructions

New Students: Submit Online via the secure DS Student Portal.

Use the DS Portal to submit your disability documentation and complete a brief intake webform about your request.

  • You’ll need your NinerNet username, password, and Student ID to log in.
  • If your documentation isn’t ready, you can indicate that in the form and send the documentation separately.
  • Please upload your documentation directly through the Portal. Submitting it elsewhere may cause delays.
  • If we receive your documentation before you complete the intake form, we will email you a reminder to complete the form.

Other Submission Methods

Students who already have files with Disability Services won’t be able to do an additional direct upload to the DS Student Portal at this time. When you have additional documentation to submit and online submission isn’t possible, we have several submission options:

  • Need to keep your originals? We’ll make a copy and upload the documentation for you.

Important Information for All Submissions:

  • Include your UNC Charlotte student ID number and University email on your documentation. Missing this information will delay processing.
  • Keep copies of all submitted documents for your records.
  • Help Safeguard Documentation: Do not include your Social Security Number (SSN). Only your UNC Charlotte student ID number and University email address are necessary for processing.
